This would be helpful if you attended a weekend at one community but live closer in proximity to the
other community and its activities.
Example: You went through a Music City Tres Dias weekend but live on the north side of Nashville, or you went through
a Middle Tennessee Tres Dias weekend and live on the south side of Nashville.
This form could also be used if you have moved from another state and wish to be added as a member to the Middle Tennessee Tres
Dias community.
